I, the undersigned, am the parent or legal guardian of the above named child. I hereby grant Colors Oh My/Flower Mound Art & Music, located at 3201 Cross Timbers Road Bldg 2 Suite 250 Flower Mound, TX 75028, the right and permission to take photos, videos, or audio recordings of my child in connection with classes, events, and other school activities. I authorize the use of these images and recordings for: educational and promotional materials, school social media pages (Instagram, Facebook, etc.), website, newsletters and flyers, classroom displays and portfolios.
I understand that these materials may be used without compensation to myself, and that I may revoke my consent in writing at any time, though previously published content cannot be removed retroactively. This release applies globally and for an indefinite time unless revoked.
I also understand that my child may in some way come to injury by use of (but not limited to) the following: art and music related equipment or tools, stools, chairs, tables, or other items, and in no way hold Colors Oh My/Flower Mound Art & Music or any of their representatives, employees, or independent contractors in any way responsible should my child come to any manner of injury whatsoever.
